2009 - New forms !
It is simple to become a RITC fighter. Get your meds done after Dec 1, 2008 and your AZ license will be good through Dec 31, 2009.
You will need:
Physical - download forms below Opthalomogical Exam - download forms below HIV - just need lab report ( no form ) Hep B & Hep C - just need lab report ( no form ).
The Axiom Group: http://www.hivandstdtesting.com will perform all three tests for $129 total and will provide them the results in 1-2 business days.
The labs are open Monday – Friday but Saturday appointments can be made by calling them before 5pm on Friday. The fighters must mention that they were referred by “MCDPH” to get this price.
Must fax to the Commission at least two weeks prior to fight day ( TELEPHONE (602) 364-1721 FACSIMILE (602) 364-1703 )
Bring your original paperwork on fight night and turn in to the Commission, (keep at least one copy for your records). Save and print from your computer.

If he's able to get a fight out here i got a buddy that will kick him some sponsorship money. His family owns a home improvement company and have sponsored local boxers (Michael Carbajal) and some mma fighters (Homer Moore)

I just went through this exact process this last month for my fight last saturday. 1, if you have a PCP you can get the physical and blood tests done for like $25. If you have eye insurance you can get the eye test done for $10 (pm me and i will hit you up with a correct eye doc in scottsdale). So instead of $129 you can get it done for $35. If you dont have the insurance then yeah go with that other place. 2, You do not have to bring the original paperwork, just fax the info to the commission and then call them and ask if it came through clear enough and they will tell you you dont need to bring it with you on fight night.
