I'm far from an expert in EOD matters, but my best guess is something went awry in prepping the munition for detonation. Someone must have miscalculated something. I would highly doubt the fatality occured after it was actually fully prepared for a controlled detonation. If only @Me_titanium_shinbone was still around. Very inconsiderate of him to vanish again.

I've read some car factory people did that and shut the lights at the end of the day and went home leaving the activists begging for help or water
