I dislike the term extreme, but of the sports that you are referring to, I have done these:
Snowboard, Skateboard, BMX(freestyle), BMX(racing), Street Luge, and a little MotoX(freestyle)
I have been planning a trip to go sky diving but have yet to make it out. I have heard it is quite the rush.

i am an avid fisherman here in the so cal/ long beach area. We generally target sharks and leave to sea in the middle of the night. Last year my crew and I caught a few over 200 and one over 300. They are absolute monsters to negotiate at the boat. One mistake and you are in alot of trouble, and hours away from a hospital.
I have parachuted off a static line 12 times at Fort bragg, NC. while serving with the 37th Engineer Batt. also waded across the river border between Turkey and iraq in possibly mine laced waters(they told me, be careful, we hear there is mines out in that water...Good luck private) for a bridge recon mission. Then I get on top of the bridge and begin measuring.......and the Turkish troops draw there weopans cause we crossed over to there sides. I damn near wet myself cause my weopan was slung over my back while i was using a tape measure...would have got mowed down...
bunji, skydiving, wake boarding, snow boarding, white water rafting, scuba with sharks, mountian climbing, rock climbing, down hill mountain biking, 150mph rollerblade hookups, et.........
free dive spear fishing (snorkel & a 3 prong) at night off Oahu's north shore, not really extreme until sharks start hitting the bag full of fish your dragging in; saw a 12' tiger between me & the surface one night, it didn't hassel me but my friends said I looked like Jesus walking on water on my way back in
